Join our busy Disabled Adults Team to deliver a service to a diverse locality across Hertsmere and St Albans

About the role

You will lead and manage a Social Care Team of community nurses, occupational therapists, community care officers and social workers. You will ensure that the team maintain a Connected Lives and positive risk strengths-based approach to improve outcomes for people through the completion of timely and effective Care Act assessments and support for carers. Deliver longer term involvement when required, crisis responses and a duty function.

You will have oversight of the locality budget and commit to deliver the Connect and Prevent initiatives.

You will provide feedback to senior management on performance and ensure the team receive sound professional support and development.

You will possess sound knowledge and a demonstrable ability to effectively apply legislation, over see Court and complex work and safeguarding in day-to-day practice.

About you

Essential:

  • A qualified and registered social worker, community nurse or occupational therapist
  • Full driving licence and access to a personal vehicle for travel across the locality
  • Proven experience in managing adult social care teams
  • Strong crisis management, assessment and decision-making skills
  • A passion for supporting people to reach tier full potential
  • Deliver services within budget and effective budget control
  • Excellent communication and partnership-building abilities
